The usual cause of the problems you're describing is a wrong type of file or a naming error on the file. For example, your file can't be mysong4/7/11.jpg it would have to be something like mysong4_7_11.jpg

So, double check the naming and file type (which is listed on the upload page) and then let me know if that fixes the problem.
